Home health monitoring includes services related to managing a chronic condition, such as diabetes, monitoring medications, as well as skilled care services, which are given by licensed medical professionals like a nurse or therapist.
Examples of home health monitoring services can include:
- Medication management, like setting up pills for the upcoming week or giving injections as prescribed by a physician
- Physical therapy, occupational therapy, or speech therapy in the home, as prescribed by a physician
- Education about living with a chronic disease, such as diabetes or COPD
- Disease management and support
- Support with personal care tasks, like bathing, grooming, or dressing

- 15-30 minute check-ins to ensure safety and wellness
- Companionship, or social, visits to decrease the risk of isolation and loneliness
- Support with personal care tasks like bathing, grooming, or dressing

Shared Other Care
Taking care of a beloved pet can be more challenging if you or your loved one have decreased energy or mobility. Shared pet care can offer the services that will keep the family pet feeling their best!
Examples of pet care visits can include:
- Taking the dog for a walk
- Changing and cleaning the litter box
- Feeding and watering the pet
- Helping with pet delivery packages, like bringing in the big box of mail-order pet food from the front porch

Examples of household management services can include:
- Grocery shopping
- Meal preparation
- Light housekeeping
- Laundry and linen services
- Transportation
